If you have not received a Claim Packet and wish to register as a potential claimant, you must first complete a Registration Form to provide additional information. Enter your email address below to be sent a link that will allow you to upload your completed Registration Form.

The Administrator will review your submission, determine whether you are eligible to participate in the Program, and notify you in writing of the determination. If the Administrator determines you are eligible for the Program, a Claim Packet, with unique Claim ID, will be sent to the email address used to register.
